STEP ONE
Recognize the need for senior care for yourself or your loved one.

STEP TWO
Call Hellenic Foundation at 773-631-5222 to help you locate your local area Illinois Department on Aging office (Care Coordination Unit –CCU) to complete an Intake/Referral and determine eligibility for free In-Home care.

STEP THREE
Illinois Department on Aging’s guidelines for eligibility:

60 years or older

Resident of the State of Illinois
-
Meet low income requirement: less than $17,500 per individual or $35,000 per couple

Apply/Eligible for Medicaid
STEP FOUR
Upon determination of eligibility for free care, CCU carries out an in-home assessment to determine the needs of the senior. Service hours are awarded accordingly.
